Checklists you can actually use
These are platform checklists to help you prepare — they are not official government forms. Official forms must be obtained from the relevant authority or court.
Platform template, not an official form. Where an official form exists, use the version published by the relevant authority.
Written complaint to the police
- Your name, address and contact number
- Date, time and exact place of the incident
- What happened, in plain chronological order
- Names or descriptions of the people involved, if known
- List of documents, photos or messages you are attaching
- Keep a copy and ask for an acknowledgement
Road accident evidence
- Photographs of vehicles, road position and visible injuries
- Registration numbers of all vehicles involved
- Names and phone numbers of witnesses
- Hospital records, prescriptions and bills
- Insurance policy details and any police paperwork
Unpaid salary or wrongful termination
- Appointment letter and any contract
- Payslips and bank statements showing salary credits
- Emails or messages about pay, leave or termination
- Attendance or duty records if available
- Any resignation or termination letter
Online financial fraud
- Transaction IDs, UPI IDs, account numbers and amounts
- Screenshots of the chat, call log, SMS or email
- Bank complaint reference number and time of your complaint
- Acknowledgement from the official cyber crime portal
Before you meet a lawyer
- A one-page timeline of what happened
- Copies (not originals) of all key documents
- Any notice, summons or court paper you have received
- A written list of the questions you want answered
